Working in collaboration with physicians and multidisciplinary team members, the Care Team Nurse (NCT) provides general nursing care and primary care team leadership while reporting to the Director of Health Services. The primary responsibility of the Care Team Nurse is to facilitate the restoration and promotion of patients’ health through the provision, delegation, evaluation, and coordination of nursing care for all individuals. Essential Functions:

• Supervises, plans, directs, and evaluates comprehensive nursing care as determined by the patient’s age-related physical, psychosocial, and cultural needs and by the policies, procedures, and standards of the health center.
• Promotes patient autonomy by facilitating the development of care objectives.
• Assists the provider during examinations and procedures, as requested.
• Provides support in the administration of cases about complex patients with substantial needs by assuming the roles of patient advocate, patient resource, and treatment coordination liaison.
• Possible duties encompass the coordination of health programs in collaboration with external organizations and agencies, the provision of health-related counseling to families, communities, and individuals regarding the prevention of communicable diseases, and the dissemination of general health-related instruction and information.
• Conducts an evaluation and assessment of the patient’s response to treatment and medication.
• Preserves accurate patient records in strict adherence to confidentiality protocols.
• Supervises the preparation and distribution of therapeutic injections, immunizations, and vaccines with precision.
• As required, offers assistance in the investigation and study of communicable diseases.
• Coordinates referrals to IHSS.

• Notifies the county of the presence of any communicable diseases.
