Working in collaboration with physicians and multidisciplinary team members, the Care Team Nurse (NCT) provides general nursing care and primary care team leadership while reporting to the Director of Health Services. Essential Functions:

  • Supervises, plans, directs, and evaluates comprehensive nursing care as determined by the patient’s age-related physical, psychosocial, and cultural needs and by the policies, procedures, and standards of the health center.
  • Promotes patient autonomy by facilitating the development of care objectives.
  • Assists the provider during examinations and procedures, as requested.
  • Provides support in the administration of cases about complex patients with substantial needs by assuming the roles of patient advocate, patient resource, and treatment coordination liaison.
